Backpack Program Application

Complete the form below before Friday, Aug. 2, 2024. If you prefer to send the school supply list as an attachment, please email it to kbedel@mercycare.org with your child's name. Backpacks will be available for pick up in the EFY Office on the first Floor of Hall-Perrine Cancer Center from 8 a.m. to 4:30 p.m. on Monday, Aug. 12.
