Schedule an Appointment
We look forward to meeting with you and/or your family members. If you have any questions regarding obtaining a second opinion or referral to our office, or any other questions, please call (319) 363-8303 locally, or toll-free at 1 (888) 400-6262.
Physician Referrals
Patients are generally referred to Hall-Perrine Cancer Center by their family physician or surgeon. Your physician should give you choices as to where you would like to receive your care. It is important that you feel comfortable and at ease when making your healthcare decisions. We believe that it is your care, your choice.
Self-Referrals
We also accept self-referrals if you have a confirmed diagnosis of a cancer or blood-related disorder. If you make a self-referral or are referred to our office, it is pertinent that we have the following information available prior to you being seen:
- Pathology report
- X-rays and reports
- Lab results
- Hospital notes/Surgical reports
- Office notes from your physicians
What to Expect
Upon your first visit to our office, you will be greeted by our receptionist who will copy your insurance cards and driver's license and have you complete necessary paperwork including:
- HIPAA Acknowledgement Form,
- Assignment of Benefits Form, and
- a New Patient Questionnaire.
If your appointment was scheduled in advance, you may have received this information in a packet from our office in the mail. If you received this information ahead of time, please bring completed paperwork with you to your appointment.
You will next meet with our Financial Counselor. She meets with all of our new patients to verify demographic information, as well as discussing your insurance benefits. We feel that this is important to make sure that your insurance claims are paid in a timely manner, as well as answering any insurance questions you may have. After your questions are answered, she will return you to the waiting room.
A member of our friendly nursing staff will then take you back to an exam room to prepare you for the physician. Your family or friends are also welcome to come into the exam room if you choose. She will obtain your vitals, height and weight. She will also go over your current medications. Please bring your current medications with you or have a list of your current medications available.
Next you will meet with the physician. Please feel free to ask questions during any point of your appointment. It may be a good idea to write down your questions before your appointment and bring them with you. You are also welcome to contact the office after your appointment with any questions that you may have. Your first visit can last between one and two hours.
