Survivorship Program
Our survivorship program is a continuation of your cancer care. The goal is to help patients navigate next steps after cancer treatment. You will be seen by our survivorship nurse practitioner who works directly with your medical oncologist.
The purpose of a survivorship visit is to:
- Talk with you about your medical history.
- Provide an individualized cancer treatment summary and care plan.
- Review any side effects you are experiencing as a result of treatment.
- Order testing such as scans or blood tests, if needed.
- Prescribe medication, if needed.
- Recommend screening tests for other cancers.
- Recommend healthy lifestyle behaviors.
- Refer to another healthcare provider, if needed.
At the conclusion of your visit, your nurse practitioner will talk with you to determine whether future survivorship visits are needed and will provide an update to your medical oncologist.
*As a continuation of your care, survivorship visits are billed to insurance.
